چکیده

Abstract Background Pulmonary vein isolation (PVI) is the cornerstone of catheter ablation therapy for atrial fibrillation (AF). However, even with important technological improvements, AF recurrences are common in long-term follow-up. Recently, epicardial fat tissue (EFT) has been proposed as a potential modulator promoting and sustaining arrhythmogenic milieu. Aim In this retrospective single-centre study, we sought to evaluate association between CT-quantified EFT volume primary PVI outcome assed by means continuous ECG monitoring implantable loop recorders (ILR) paroxysmal patients. Methods A cohort 92 patients (median age 60.2 [IQR 51.9–64.0]; 74% male) (a single persistent episode was accepted) undergoing targeted at analysed. Prior CA all underwent cardiac CT imaging subsequent implantation recorder (ILR). Radiofrequency electro-anatomical mapping used 79 patients, balloon cryoablation 13 Verified complete defined procedural endpoint. semi-automatically segmented quantified on contrast enhanced studies. recurrence, burden >0.1% after blanking period (90 days), continuously assessed ILR. Kaplan-Meier analysis recurrence-free survival 1-year, 2-year 3-year follow-up periods compared stratified into tertiles. Results During median 52.6 months 43–64], recurrence detected 58 (63%) 29 (50%) them repeat ablation. Patients high, mid, low total tertiles had time late 154, 373, 284 days (Fig. 1A). At 1-year follow-up, high tertile worst (p=0.023). difference gradually faded-out became non-significant 2 3 years. ROC revealed an 95 ml optimal cut-off (p=0.002) expected arrhythmia post acutely successful 1B). Conclusion quantification relatively homogenous, monitored significant 1 year fat. These results suggest possible role adipose adverse pro-fibrillatory factor. Identified cut-offs have be validated prospectively larger balanced patient population. Mechanisms and clinical significance of early recurrences of atrial arrhythmias after catheter ablation for atrial fibrillation

Early recurrence of atrial arrhythmias (ERAA) after ablation is common and strongly predicts late recurrences and ablation failure. However, since arrhythmia may eventually resolve in up to half of patients with ERAA, guidelines do not recommend immediate reintervention for ERAA episodes occurring during a 3-mo post-ablation blanking period. Certain clinical demographic, electrophysiologic, pro...

Monitoring Atrial Fibrillation After Catheter Ablation.

Although catheter ablation is an effective treatment for recurrent atrial fibrillation (AF), there is no consensus on the definition of success or follow-up strategies. Symptoms are the major motivation for undergoing catheter ablation in patients with AF, however it is well known that reliance on perception of AF by patients after AF ablation results in an underestimation of recurrence of the ...

Prevalence of asymptomatic recurrences of atrial fibrillation after successful radiofrequency catheter ablation.

INTRODUCTION The long-term efficacy of radiofrequency catheter ablation of atrial fibrillation (AF) has been based on patient-reported symptoms suggestive of AF. However, asymptomatic recurrences of AF may remain undetected. The aim of this study was to determine the prevalence of asymptomatic recurrences of AF after an apparently successful catheter ablation procedure for AF. METHODS AND RES...

Left Atrial Appendage Volume and Plasma Docosahexaenoic Acid Levels Are Associated With Atrial Fibrillation Recurrence After Catheter Ablation

BACKGROUND Risk factors for atrial fibrillation (AF) recurrence in patients who have undergone AF catheter ablation have not been fully clarified. The objective of this study was to assess whether the left atrium (LA) and LA appendage (LAA) volumes, and cardio-metabolic markers such as polyunsaturated fatty acids (PUFAs) levels were associated with AF recurrence. METHODS Seventy-seven consecu...

Dofetilide is safe and effective in preventing atrial fibrillation recurrences in patients accepted for catheter ablation.

AIMS The aim of this study was to assess the safety and efficacy of dofetilide among patients refractory to other anti-arrhythmic drugs (AADs) and accepted for atrial fibrillation (AF) ablation. METHODS AND RESULTS One hundred and twenty-seven of 454 patients (69% male, 58% paroxysmal, age 60 +/- 10 years, AF duration 8 +/- 7 years) scheduled for AF ablation between February 2004 and May 2008...
